Eligibility

To qualify for most of our programs, an individual must be a Lake County resident, have very low income, and they or one of their children must have a serious mental illness. Extended Housing, Inc. programs have different sets of eligibility criteria. Please see the description of our programs for more information.

Referrals

Although most of our programs do not require a referral, we accept referrals from providers, including those at Signature Health and Crossroads Health. For more information on the referral process or to refer a client to our services, please call 440-352-8424 ext. 110, and leave a message.

Please note that participants in the Shelter Plus Care program are referred through the Lake County Coordinated Entry through the Lake County Continuum of Care.
